New Paperwork Policy

We understand doing paperwork is a hassle, but it is a necessary part of running a medical practice. You will be asked to completely update one time a year and verify at each visit with showing your ID and driver’s license. The paperwork doesn’t just to update your personal information, it’s your medical history, prescription updates, HIPPA, permission to treat you and file your insurance. Paperwork is available on our website and we can send it to you via portal, email or fax.

If you refuse to complete paperwork then you will be ask to reschedule your appointment till it is done.
